A request, not a confirmed booking
Submitting the form at Book Appointment sends us a request. Your appointment is confirmed only after our team contacts you by phone, text, or email and agrees on a time with you. Requested dates and times are not guaranteed.
When we respond
We review requests during regular office hours and aim to respond within one business day. Requests sent in the evening, on weekends, or on holidays will be answered the next business day. If you have not heard from us, please call (310) 553-2233.
Not for emergencies
Please do not use the online form for urgent problems. For a dental emergency during office hours, call us right away. If you have severe swelling, trouble breathing or swallowing, uncontrolled bleeding, or facial injury, call 911 or go to the nearest emergency room.
Accurate information
Please provide accurate contact and insurance details so we can confirm your visit and verify benefits. Requests for patients under 18 must be submitted by a parent or legal guardian, who should attend the first visit.
Privacy of your request
- Your request is sent over an encrypted (HTTPS) connection and delivered only to our office.
- Information you provide is used to schedule and prepare for your care, and is protected under our Notice of Privacy Practices and Privacy Policy.
- Please share only what we need to schedule you. Detailed health history can be completed at your visit or discussed by phone.
- Advertising and analytics tools never receive the health details in your form.
Consent to contact you
By submitting a request, you agree that we may contact you by phone, email, or text about that request. Ongoing appointment reminder texts are sent only if you opt in, as described in our SMS Terms.
Changes, cancellations, and fees
Once your appointment is confirmed, our Cancellation & No-Show Policy applies, including the 48-hour notice requirement. Payment is handled under our Financial Policy.
New patient offers
If you request an appointment through a promotional offer, the offer terms apply and will be reviewed with you when we confirm your visit.
