We want you to get the most from your dental benefits. This policy explains which plans we accept, how we work with your insurance company, and what you are responsible for.
Plans we accept
- All PPO plans accepted. We work with all PPO dental plans.
- Accepting some HMO plans. We accept some HMO (DHMO) dental plans. Please call us to confirm whether your plan is accepted. Some HMO plans also require you to select our office as your assigned dentist before your visit, so please call us and we will help confirm.
- Medi-Cal and Denti-Cal are not accepted at this time.

Before your visit
Please share your insurance information when you request an appointment, and bring your insurance card and a photo ID to your first visit. We will verify your eligibility and benefits before treatment whenever possible.
Claims
As a courtesy, we file claims with your dental plan for you. We ask you to keep your information current, including any change in employer, plan, or subscriber.
Your responsibility
- Your dental insurance is a contract between you and your plan. Our office is not a party to that contract.
- Any benefit estimate we provide is based on information from your plan and is not a guarantee of payment. Your plan makes the final decision.
- You are responsible for deductibles, copays, coinsurance, waiting periods, frequency limits, annual maximums, and any service your plan does not cover.
- If your plan has not paid within 60 days, or denies a claim, the balance may become your responsibility. We are happy to help you follow up with your plan.
Pre-treatment estimates
For larger treatment, such as crowns, implants, or Invisalign, we can send a pre-treatment estimate to your plan. This can take several weeks. You may choose to wait for the response or proceed, knowing that the estimate is not guaranteed.
Secondary insurance
If you have coverage under more than one plan, we will file with your secondary plan after the primary plan pays, following coordination of benefits rules.
Cosmetic and elective treatment
Many plans do not cover cosmetic procedures such as teeth whitening or veneers. We will let you know when a service is likely not covered so you can make an informed decision.
Promotional offers
New patient specials and other promotions follow the terms listed with each offer. Unless stated otherwise, promotional pricing cannot be combined with insurance benefits.
